私は医師でも医療関係者でも栄養士でもないが、証拠があるので安心して医師の誤りを指摘できるというわけだ。
実のところ、ベジファーストが食事摂取基準から削除されたというのは正確な表現ではない。
2020年度版では、ベジファーストについて一文取り上げていた「食事摂取パターン (eating pattern)とシフトワーカー」という節が糖尿病対策の章からまるごと削除されたのである。
食事摂取基準の策定根拠は健康増進法である。法に定められた食事摂取基準の取り扱う事項は以下の通り。
栄養素というのは、もちろんたんぱく質とかビタミンとかミネラルとかのことを指すので、野菜の食べ順なるものはOut of Scopeなのだ。だから削除されたそれだけである。

でも、それもある程度は仕方ない。2020年版で494ページですからね。鈍器本のページ数です。忙しい栄養士が横着して基準値だけ見る、そして基準値が独り歩きしだす。偉い先生はこれがホント嫌なわけです。
しかしエビデンスというのは毎年増えていくわけで、理屈で言えば毎改訂ごとにページ数は増加します。今でさえガイドラインを読むのは大変なのに、これ以上！となるのは大変ですよね。

実際、2025年度の検討会資料を読み込んでいると「人手不足でもはや持続困難だ」っていう研究者の悲鳴がかなり伝わってきます。
そんなこんなで食事摂取基準をこれ以上肥大化させることは避けねばならないってのが2025年度改訂の問題意識の１つだったわけですわ。
日本では、政府の作る食事摂取基準以外にも、EBM（エビデンスベースドメディシン）に基づく多種多様な診療ガイドラインが存在します。これらは専門の学会が作っとるわけですが、ここにも「食事療法」という節はあるわけですね。
栄養素の基準値策定と、食事療法。かなりカバー範囲が重なり合っています。政府と学会、お互い独立に作っているものですから、似たような記述がお互いのガイドラインで執筆されるという無駄が生じます。さらに悪いことに、違う人が作っているわけだから、食事摂取基準と診療ガイドラインで矛盾が生じるという事例もありました。当然、現場は混乱しますね。
食事摂取基準と診療ガイドラインで連携して、お互いの責任範囲を排他な形で定めよう、縄張りを線引きしよう。そうすれば読み手の負担も書き手の負担も軽くなる。そういうことになったんですね。
偶然、糖尿病診療ガイドラインの改訂作業は、食事摂取基準2025年度版と同時期に行われていました。糖尿病診療ガイドラインの食事療法を2024年度版と2019年度版とで比較してみましょう。
2019年には13あったQ＆Aが2024年には8に減っています。
2019年にはあった、栄養素絡みのQ＆Aが軒並み削除されています。
理由はここまでの説明で明らかですね。食事摂取基準の縄張りを尊重したのです（正確に言えば、「総エネルギー摂取量」の記述はまだ問題が残っているのですが……）。
さて大いに脱線した話を戻しましょう。食事摂取基準の糖尿病に関する記述から、ベジファーストがから削除されたのはOut of Scope、範囲外であるためでした。となると、執筆責任は糖尿病診療ガイドラインの食事療法にあるはずです。
あれ？ 2019年にはあったベジファーストが、2024年版糖尿病診断ガイドラインでも消えてるぞ？ なんでやねん。
最初に筋肉博士医師のツイートは、「真か偽かで言えば端的に誤りであるし、好意的に読んでもミスリード」と書きました。「好意的に読んでも」と書いたときに想定していたのは、「食事摂取基準」を「日本の糖尿病ガイドライン」と読み替えれば仮説としてはありうるだろう、という意味でした。
しかし読み替えたとしても、「エビデンス不足がようやく露呈」というのはミスリードな表現だと私は考えます。
糖尿病診断ガイドラインは食事摂取基準と違って、学会で作られているので改訂方針について外部の人間がアクセスできる情報がありません。どういう意図で情報が削除されたのかさっぱりわからないんですね。削除の理由をすぐに「エビデンス不足」に断定するのは証拠がありません。
しかも、糖尿病診断ガイドライン2024年版で検索すると出てくるニュース記事、
を読むとですね、副題が「糖尿病の新診療ガイドラインに見る食事のポイント」で、解説者がガイドライン策定委員会に所属する食事療法の専門家（京大教授）であるにも関わらず、新ガイドラインでは記述が削除されたはずのベジファースト/カーボンラストについて解説が行われているんですね。
内容は「世間で言われるベジファーストには誤解がある」というものですが、大雑把に言えば食べ順を考慮して食べることを推奨する解説で、2019年の旧ガイドラインに準拠していると読めます。つまり「ベジファーストが誤りだったから削除された」わけではないことを示唆しています。
じゃあなんで削除されたのか？ 私は別の仮説を提示したいと思います。
2019年ガイドラインと2024年ガイドラインを交互にじっくり眺める見てください。
2024年版の方が、Q＆Aの問いの文が相当洗練されているんですよね。
診療ガイドラインというのは、「Minds診療ガイドライン作成マニュアル」という規範となる規格がありまして、Q＆Aをどう書くべきかというのも死ぬほど細かく厳密に定められています。
細かい事抜きに、マニュアルにどういう要件が書かれているかということを超雑に述べると、まず問いは「臨床上生じる意思決定に関する疑問」でなければなりません、そして回答はガイドライン策定者自身が実施するシステマティックレビューに基づくものでなければなりません。
これ前提に2019年版の問いを見返してみると「Minds診療ガイドライン作成マニュアル」に明確に非準拠ですね。「〇〇はどう影響するか」系の問いはどう見ても意思決定に関わるものと言えません。
そんなわけで、2019年から2024年の改訂方針には、「Minds診療ガイドライン作成マニュアル」への準拠度を高めるという意図があったと見て取れます。
次に、2024年版で削除された、2019年版のベジファーストに関するQ＆Aを見てみましょう。
これベジファーストだけに関するQ＆Aではないんですよね。小ネタのアラカルト的な回答で、複数の問題をごちゃまぜにしているという印象を受けます。
そんだけ2019年は自由にやっていたというわけですが、「Minds診療ガイドライン作成マニュアル」に準拠するかたちで、問いを書き直そうとするならば、問題を分割する必要があるでしょう。
試しに書き直してみると、こんな感じでしょうか。
ただ、2019年版のガイドラインでは、小ネタのアラカルトとして立項のバリューを出したわけで、わざわざ3つに分割して3つとも立項するだけの意味があるかというのは難しい問題です。
「Minds診療ガイドライン作成マニュアル」にもこう書かれています。
日常診療において医療行為を選択する意思決定の場面は数多くあるが， その全てについてシステマティックレビューを実施し， 推奨を作成するのは非常に多くの労力を要するため，現実的ではない。そのため，診療ガイドラインでは実臨床における問題に対する回答を導き出したいものを厳選して重要臨床課題として取り上げることが望まれる。
3つともに、「炭水化物制限の推奨」とか「食物繊維摂取の推奨」とかに比べれば、超小ネタですので、臨床課題であっても重要臨床課題ではない、というところで落とされたのでは？、などと私は想像しています。
書籍『佐々木敏のデータ栄養学のすすめ』などでも解説されていますけど、野菜先食べを厳密に実行するべきとするエビデンスは、おそらくまだ存在していないと思われます。
ただ、食事の楽しみを損なわない範囲で、炭水化物系を食べるのを後回しにするのは、それなりに有望な心がけと言ってもいいじゃないでしょうか。また野菜先食べである必要はなくて、たんぱく質先食べでもいいという論文もあります。
この年度の末で学校教諭を辞めている。ラストイヤーである。思えばあっという間だった。上の秋季大会の後も、大なり小なり大会があったが、目立つ成果はなかった。
実力というよりは、柔道部全体の精神力、やる気、気合いの問題だった。センスがありそうな子はいたが、『本気』になり切れない。なぜかはわからないが、若者特有の殺気に満ちた闘志というか、「目の前の相手を殺してやる」という滾(たぎ)りが見られない。
特に、格上相手に戦う場面がそうだった。勝つとか負けるとかじゃなく、全力でぶつかってみせようという気概を感じない。
そんな中で、Ｔ君はじわじわと実力を伸ばしていた。この頃だと、ベンチプレスは140kgを上げるようになっていた。
柔道の動きはボテボテとした感じだったが、それでもサマになってきていた。高校から柔道を始めた関係で、高一の頃が特に弱かったらしい。当時は、毎日のように先輩や同級生から千切っては投げられていたという。
シゴキ以外にも、いろいろあったようである。当時の先輩連中からは相当イジられていたらしい。Ｔ君の同級生の柔道部員が、確かこんな内容を回想していたっけ。
「おいチクビ。何を寝とる。はよ畳の上から立て!!」
「今、こいつに筋肉ドライバーしてやったら、首めっちゃ抑えて転がっとるしwwwww」
「いつまで畳で寝とんな。早う起きんとチクビねじ切るどっ！」
「チクビ君さあ。今度の公式大会の時に、絶対シャイニングウィザードしてよ。武藤敬司好きなんやろ」

「プロレス好きなんだって？これからダブル大外刈りするから付き合ってくれ」
彼が弱かった頃は、部内でもやられたい放題だったようだ。しかしこの頃、高三の手前になると、彼は部内で№1になりつつあった。皆、彼に一目置くようになっていた。当時、部のキャプテンだった100kg超級の子と乱取りをしても吹っ飛ばされない。
それでも基本的には、いじられキャラだった。先輩からも後輩からもいじられていた。しかし、彼はどんな時でもリアクションを欠かさず、スマイルで彼らに応えていた。
そういえばＴ君は、毎日必ず昼休みに柔道場に来て、一人で弁当を食べていたっけ。一人飯には理由がある。弁当を食べた後に、ヤツは筋トレを始めるのだ。ベンチプレスを始めとして、腹筋に、背筋に、あとは自転車のゴムチューブを柱に括り付けての打ち込み練習、あとはシャドー柔道か。さすがにカマキリは相手にしてないと思うが……。
昼休みに柔道場に行った折、彼を何度も見ている。ベンチプレスが一番好みのようだった。自分自身が成長する実感とでも言えばいいのか、月ごとに最高記録が上がっていくのが面白かったのだろう。
柔道界の公式回答としては、高校生に器具を用いた本格的な筋トレをさせるべきじゃない。この時期(小中～高校生)は、技術に磨きをかける時期だ。ベンチプレスその他で測れる筋力は、試合ではそんなに使わない。柔道に本当に必要なパワーというのは、柔道をすることでしか身に付かない。すなわち組み手の練習とか、立技・寝技の乱取りを通じてしか、そのスポーツに必要な『体力』は身に付かない。
しかし、Ｔ君には時間がない。彼にとっての柔道は、おそらく高校で終わりである。時間がなさすぎる。だったら、ひたすら筋トレに励む道があってもいいんじゃないか。当時はそんなことを考えながら、昼休憩中のＴ君のトレーニング風景を眺めていた。
高三の四月時点で、彼の体重は約78kgだった。一年前はボンレスハムのような体形だったが、スマートになっていた。何より、当時のＴ君は部内で一番やる気があった。何事にも一生懸命だった。鍛えがいがある。
さて、高三最後の公式戦は六月だった。ここまでには仕上げたい。幸い、四月にあった市内大会では、彼は５位入賞を果たして県大会出場を決めていた。順々決勝では、昨年の秋季大会で当たった強豪校の選手に内股で一本負け（空中一回転半させられた）だったが、そこから２回勝って５位になった。
柔道場での立ち技の乱取りでは、納得いくだけの投げを打てるまでひたすら稽古を続けた。Ｔ君に対し、組み手の位置は自由にさせて、技も全部受けてやる。その代わりクオリティは一切妥協しない。俺が納得いくまで休憩すらさせない。
「おい、Ｔ。苦しくっても、苦しそうな顔すんな。相手にバレんぞ。ビビってるとな、わかんだよ。相手によ～!!」」
すると、唇を引き締めるようにしてＴ君は、距離を取ってこちらの様子を伺っている。
ああ、これは。技に入るのを狙ってるな。相手が高校生だとすぐにわかる。
「技に入るのを狙うな。狙わなくていい。(お前の反射神経だと)無理だから!! とにかくたくさん、技をかけろ。連続しろ、連続。ちゃんと技は繋がるから。お前の感性でいいんだよ。考えるな、感覚でやれ」
実際、高校レベルの試合だと技が下手くそでもいい。とにかく数。連続性が大事だ。
ただやはり、Ｔ君は運動神経が鈍いようで、へっぽこな大内刈りに小外掛けや、ボテボテとした払い腰しかできなかった。ヤツが俺に珍しく内股をしかけると、「ポフッ」という効果音とともにヤツが弾かれる。そんなレベルだった。素人未満である。
「今のお前の柔道、ほかの選手と乱取りやってるのと違うじゃん。俺用の柔道か～、そのへっぽこなのはよ～」
Ｔ君の柔道は、俺と乱取りしてる時はモードが違ってる。まるで、何かの専用の練習でもしてるようだった。
ヤツはまた内股をしかけてくるが、全然効いてない。運動神経とか、技に入る姿勢の問題じゃなくて、組み手が下手くそ（≒柔道着の握る位置がおかしい）なのだ。いいところを掴めてない。せっかく自由に組ませてるのに。
ただ、Ｔ君に組み手争いを教えるには、もうタイムオーバーである。「思いっきりイイと感じるところを掴め！」くらいしか教えていない。
だが、連続して技を掛け続ければ可能性が見えてくる。うまく相手が転がれば、彼が得意とする寝技に移行するラインも見えてくる。
「オーイ、てら。何止まってんだよ！ 動け、動け。変数作れ。チャンスがないと思ってんの、お前だけだよ」
彼はヘロヘロになって大内刈りを放つのだが、俺に効くはずもなく。間隔を取って、釣り手と引き手（※柔道用語は適当にググってほしい）を上下にブンブン振り回したなら、ジャイアントスイングみたいになって、Ｔ君が畳をゴロゴロ転がっていく。築地市場の床面を滑っていくマグロのようだった。
「オーイ、俺お前よりも軽いんやぞ。吹っ飛ばされてどうすんだよ」
Ｔ君が立ち上がると、顔が真っ赤になっていた。熱中症の人でもこうはならないだろう。それくらい真っ赤だったよ。
ゆっくりと俺に近づいてくると、釣り手の方でパンチするみたいに組み手を取ろうとしてきた。それを弾いて、逆に奥襟を取り返してやった。すると、ヤツも負けじと奥襟を取ってくる。
「力比べで俺に勝てるか」と思ったが、ヤツも結構強かった。睨み合ってたが、Ｔ君の耳まで真っ赤になってるのを見た。
この頃になると、彼の耳は潰れていた。餃子耳というやつだ。努力の証である。大半の柔道選手は餃子耳にならない。本気で寝技を練習した証である。
こんな光景を見ながら、ある漫画のセリフを思い出していた。 技来静也が描いた漫画作品の、確かこの場面だったはず。

俺だって、中学生や高校生の頃は、練習が恐ろしいことがあった。寝技の最中に、殴られ、蹴られ、締め落とされ、「殺される」と思ったことが幾度となくある。だが、試練を乗り越えるだけの胆力を身に付けないことには、いつまで経っても二流以下である。
こういう感覚は、今時のＺ世代みたいなやつがあるじゃん。ああいう子達には伝わらないんだと思う。「○○が上手になりたい、強くなりたい」って？ 本当にそうなりたいなら、命くらい投げうってみせろ。
2024年現在だって、野球でもサッカーでも、バレーでもテニスでもゴルフでも、スケボーでもブレイキンでも、将棋だって囲碁だって、その道で超一流の奴はな、自分の命かけてその道を歩んでんだよ。
立ち技が終わると、次は寝技だ。Ｔ君が強敵相手に勝とうと思ったらこれしかない。それくらい、彼の運動神経は絶望的だった。立ち技の才能がない。当時のＴ君が「生まれつき運動神経が切断されてるんです」と言ったら、信じたかもしれない。
柔道場の端にある、寝技の乱取り開始を示すタイマー(ブザー)が鳴ると、俺はさっそく横四方固めでＴ君を押さえつけて、右拳の先を太ももに突き立てた。するとＴ君は痛がって、俺の腹を突いて逃れようとする。
「お前、俺に力で勝てんの、勝てんの？ 勝てないでしょ!? 頭使わんと、ほら。前に教えたやろ？ 逃げ方を……」
横四方固めで抑え込まれた状態のＴ君は、腕をなんとか両者の間に差し込んで、背筋で跳ねて逃れようとした……それが正しいやり方だったら、そのまま逃がしてやる。それで、また瞬間的にヤツに覆いかぶさって、抑え込みに持っていくのだ。Ｔ君は逃げる。ひたすらそれを繰り返す。
俺の抑え込みから逃れたとしても、Ｔ君がこっちに向かってこず、亀の姿勢になろうとすると、すかさず両手で奴の柔道着の胴と太腿を掴んだ。
「よいしょっ」と持ち上げると、奴の体がひと時宙づりになった。そのままクルリとひっくり返して、横四方固めでまた抑え込んだ。今の選択はアウトだ、敵前逃亡である。この寝技練習では、攻撃行動以外は認めない。
寝技になると、Ｔ君がたまに覚醒する。俺の動きがわかってたみたいに、例えば、真上から抑え込もうとした俺の膝をピンポイントで蹴っ飛ばして、転がして、そのまま上に乗ってしまう。
俺は両足をヤツの足に絡めてガードするのだが、巧みに外して、抑え込みに持っていく――縦四方固めが完成した。
こんな具合で、いい動きをした時には認めてやる。褒めるんじゃない、認めるのだ。そいつのパワー、気概、実力を。褒めるってのは、親が子どもにやるものだ。コーチと選手って対等な関係だろ。
Ｔ君は膂力(りょりょく。腕の力)があるうえに、寝技のセンスがあった。寝技というのは、ガッチリ決まると体力差に関係なく勝利できる。当時まだ20代半ばだった俺の体力はＴ君をはるかに上回っていたが、それでも抑え込みから脱出できないことがあった。
ただ、悲しいかな。彼にあるのは寝技だけだった。立ち技は本当にダメだった。まったく才能がなかった。
せっかくパワーがあるのに、釣り手で相手の奥襟や背中を掴むことは皆無であり、いつも前襟だった。運動神経がないから、せっかく神業的なタイミングで大内刈りや払い腰に入ることができても、相手を投げるに至らない。遅すぎるのだ。
ただ、彼はよくやっていた。性格やキャラクターはいじられ系芸人のそれだったけど、リアクションはよかったし、部員みんなから好かれる存在だった。そういう雰囲気をもった人間だった。
時期は半年ほど戻る。
いつぞやの試合でＴ君を内股で吹っ飛ばした名門校の選手と、ラーメン屋で同席したことがある。Ｔ君と俺と、あの選手Ｎ君が一緒になったということは、おそらく試合の帰りだったのだろう。
あれは、福山駅の駅舎の外に入っているラーメン屋だった。名前は忘れた……なんとか吉だったはず。そこで、Ｔ君にラーメンをおごってやろうとして、カウンター席がいっぱいで４人掛けのテーブル席に通された。
そしたら、おかみさんが来て、「相席でもいいですか？」と言うのだ。それで待っていたら、Ｎ君ともう１人の柔道高校生が向かいのテーブルに付いた。２on２の相席Styleである。
そのＮ君だが、痩せ気味で精悍な顔つきをしていた。体形は相当細身で、背丈は185くらいか。Ｔ君と同じく81kg級の選手である。もう１人は、100kg超級とおぼしき超アンコ体系の子だった。棟田選手以上の丸々とした感じだ。
２人とも、耳は完全に潰れていた。どちらも社会人に近しい貫禄があった。眼光が鋭い。スポーツ特待生で高校入るような奴って、雰囲気からして違ってる。
俺の耳もやはり潰れている、大学卒業して釈迦人デビューするにあたり、手術で直すという選択肢もあったが、餃子耳の方がハクがつくと考えていた。Ｔ君の耳は、この時潰れかけだった。
会話、どんなだったかな。そうだ、Ｎ君がＴ君に話しかけてきたんだ。
それでＴ君も、ラーメンを待ちつつ水を飲んで彼と話をしてた。
思い出せる範囲だと、こんな会話だった。覚えてない箇所は端折っている。
「高校入ってやっとるよ」
「そうなんだ。やっぱりこの辺の学校は、そういう子が多いのかな」
「えー、すごいやん」
「親に無理やり。両親とも柔道してて」
「それで、ここまで強くなれたんや。すごいね」
「内股めっちゃすごいやん。カミソリみたい。サクッと相手が飛んでくよね。マジですごい」
「伝家の宝刀な。あれだけは自信ある。けど、強いヤツは世の中にいっぱいいるし。俺もまだまだよ」
「自分、Ｎ君には一生敵わないな。多分。こないだの試合、内股で空中５回転しとらんかった？」
「(テーブルを叩きながら)してた、してた!! なあ～」
※隣の高校生が関心なさそうに頷いていた
「もっと勢いが強かったら、会場の窓ガラスを突き破って護国神社に落ちとったよ」
このあたりで、俺のイラつきを察したのか、Ｎ君の隣の高校生が彼を小突いた。
「Ｔ君さ。今度オレと試合するのいつになるかな」
「冬に県に繋がる個人戦がなかった？」
「それ、無差別級だけだろ。あの大会は、ベスト８までは全部うちの高校が占めるし、そこまで登ってこられる？ それにベスト８が決まったら、例年それで市大会は終わり。解散。１位から８位の順位はうちの監督が決める」
「えー、そういう仕組みなん？」
ここで、隣の高校生がＮ君の肩を叩いた。
「そう、そういう仕組みな。次の試合は春しかない。あと１回だけのチャンス」
「うん、当たるの楽しみにしてる」
「そんなんされたら、自分死ぬし!! ボンレスハムだし、チャーシューだし、畳をバウンドしちゃうよ」
「楽しみにしてる」
ここらへんの場面で注文したラーメンがきた。それで、あとは皆黙々とラーメンとか餃子を食べて、解散した。
あの店は、とんこつラーメンのアレンジ系が美味かった。当時はよく利用してた。もう軽く十年以上は行ってない。残りの人生で行くこともないのかもしれない。
※書いてるうちに思い出した。福山駅前の八十吉ラーメンである。
ただ、あの頃のＴ君とは、できれば一緒に行きたくないな。財布的な意味で。
当時、たまに２人きりでご飯をおごってやることがあったけど、あいつ、ラーメン替え玉４杯＋チャーハン＋から揚げ＋餃子５人前とか食べ切っててビビったわ……食欲が凄まじい。ラーメンが４杯で終わりなのは、その頃にはスープが無くなるからだ。
俺はその半分しか食えなかった。食欲においては、完全に俺の負けである。事実だ。認めるしかない。
そんなこんなで、最後の公式戦までの日々は風のように過ぎていった。
この匿名ダイアリーを書くにあたり、約二十年前の記憶を思い返している。正確じゃない部分はあろうけど、そこまで間違った内容でもないはず。一部ではあるが、当時の記録媒体を基にして書いてることもある。
あの日々は、けっこう幸せだった。柔道部の成績は悪かったけど、気合いが入った部員も出てきていた。そういえば、あの時そういう行為をした女子マネージャー(妹)も、あれから特にトラブルはなかった。これまでどおりの、柔道部顧問とマネージャーの関係だった。いや……ごく稀にプライベートで会うことがあったか。
何度か変なことがあった。双子の姉の方と柔道場の入口で談笑してると、妹の方がやってきて一緒に話に入ったのだ。不自然なタイミングで。無理やりに近い。それは、姉じゃないもう一人のマネージャーと話してる時もそうだった。
ちなみに姉の方は、マネ子に比べると若干明るい性格だった。俺ともＴ君ともほかの部員とも仲良くやってた。天性のコミュニケーション能力があるタイプだった。底抜けに明るいわけじゃないけど、不思議と安心感のある、温かみのあるキャラクターだった。
マネ子の態度は、上記を除いては普通だった。何か月かに一度は、俺も含めた部員みんなにお菓子を作ってくれたし、バレンタインデーには手作りチョコレートくれたし、大会前には率先して雑用を命じてほしいと志願したし、俺の誕生日にネクタイピンをくれたこともある。
私学だし、まあいいかと思ってもらってた。あの子の財布は大丈夫かなと思ったが、月のお小遣いが一万円と聞いて納得した。友達と遊びに行く時などは、母親から別途お金がもらえるらしい。医者の娘は違うな……と、月四千円の小遣いで寮暮らしをしていた高校時代に想いを馳せたっけ。
別に、妹の方とまたセックスしたいとは思わなかった。「したい」という欲求はあったけど、でも教師としての責任感が勝ったのだ。 訂正；間違った記憶でした…
ただ、俺という人間は、やはり異常だったと思う。ちょっと性欲が強いのは間違いない。そちらの方には正直だった。あとは～～子どもの頃に柔道や陸上をしてたのだが、絶対にうまくなりたい！ という情念が強くなりすぎて、体を痛めて指導者に怒られることがあった。夢や目標に対しては猪突猛進だった。
性欲、性欲……と、今思い返してみて、人としてよくなかったと思えるものを最低３つは思い出すことができた。ひとつだけ、あれはまだ5才か6才の時か。両親が忙しい家庭だったので、日常の世話は祖母が見てくれていた。いつも、祖母と実妹と一緒に寝床についていた。

間違い無くこれが最高傑作。
睡眠時無呼吸症候群と言う病気がある。寝ている途中に呼吸が止まり、血中酸素濃度が下がり血圧が上がり、と身体に無茶苦茶負担がかかる病気で、睡眠が浅くなるだけではなく色々な病気を引きおこす。
ワイの母が脳卒中になったのはこれが原因ではないかとにらんでるぐらい、重視している。（2回再発した後にこれが発覚して治療開始したらそれ以降再発なし・血圧も安定した）
しかし、実は診断がつけばわりと対処ははしやすくて、これを機械的に解決するのがこのCPAPと言う装置。人工呼吸器の一種で、高圧で空気を送り込む事で、睡眠時の呼吸をサポートする装置。これを使えば手術などはいらないのだ。
ただ、母は寝相が悪く、寝ているときにこれが外れてしまうことが悩みだった。外れると効果が無いのは当然として、かなりの騒音と、凄い勢いで空気が出てくるので冬場は寒くて目が覚めることも。
そこで、3Dスキャナで母の顔をスキャンし、3Dプリンタで顔面の形をオフセットした形をもつ、市販マスクの外側に被せて鼻から頬のあたりまであるマスクを作り、市販のマスクのゴムを3Dプリンタ製のマスクに通して母の顔に合う様にしたことでズレないようにした。
効果はてきめんで、夜中にマスクがズレて付け直すという作業が全く無くなった。締め付けも弱くて大丈夫になって母も着け心地がよくなったと評判。
メンテナンスに来たCPAPの代理店のお兄さんにも感心してもらって、メーカーから問い合わせがあって3Dデータを渡したりもしたくらい、これははっきり最高傑作だと言えます。これはガチで結構試行錯誤したし。
車椅子を乗せられる車を持っていると言ったけど、乗り心地的にはやっぱり車椅子ってそれほど良くなくて、ガタガタと揺れる場合は長距離は難しい。
そう言う時は頑張って普通の座席に母を乗せることになるんだが、そうすると今度は肘掛けに相当するものがないため、身体が倒れていてしまう。
そこで、母の身体を3D スキャンし、その形を保持する形を作って、それをポリウレタンフォームのいたの厚みでスライスしてカットパスを作って、レーザーカッターでカッティング。
さらにアクリルパネルとベニヤ板で外側を固定しつつ、捨てられていたチャイルドシートの金具をもらってきて座席に固定できるようにした。
これによって、長距離の移動でも立位を安定できるようになって、ドライブ好きの母をドライブに連れて行けるようになった。
特に認知症で不安な面が強く出る時なんかは、こうしてドライブに連れていくと安定するので役立ってる。
ただ……脆いんだよね。すぐ壊れる。うちょっと改善したいが時間がない。固い材料にすると今度は座り心地が悪くなりそうでもある。
ただ、データはあるので、壊れてもすぐにリピートできるのもデジタル化しているメリットではあるんだが。
家の中の移動では、スタンドリフトという介護器具を使って母を移動させている。リフトを使って車椅子に座らせたら、次に肘掛けに手をやらせて、その間にリフトから足を降ろすと言う事をするのだけれど、どうしても肘掛けを手探りで認識できずにぽろっと手を落としてしまうことがある。確率的には半分ぐらいなのだが、もう一日何回もトイレに着替えに食事にと繰り返すので、地味にストレスになっていた。
そこで、肘掛けの先端にハマるようにプラスチックのキャップみたいなものを作って、そこに丸い突起を付けて、ちょうど手で包み込むように握れるようにした。これで安定して車椅子の肘掛けを認識できるようになって移乗が楽になった。
そこで、3Dプリンタで、手のひらよりちょい小さいぐらいのボタンを作り、その中にSwitchbotのボタンを仕込んで、さらにそれをレーザーカッターでカットしたパネルにはめ込んで、巨大ボタンスイッチを作った。
それをキーにしてやることで、色々なものを本人が操作できるようにした。部屋のライトON/OFFボタン、テレビをONにしてNHKを出すボタン、トイレ行きたくなったら押すボタン。
（非常用ボタンは別のローカルで完結するシンプルな装置でやってます）
これは俺が考えたわけじゃなくてあちこちで出回っていたアイデアだけど、コロナ禍でマスクをしなければいけない時に、ずっと耳掛けのマスクをしていると耳が痛くなる。そこで、3Dプリンタとゴムでマスクを引っかけて頭の後ろで固定するアダプタを3Dデータをもらって作って出した。
すると、お世話になってる施設の人からもほしいと言ってもらって、大量に量産した。今は市販のやつがあるのでお役御免。
食事をするときに、食器を倒してしまったりする。一方でモテないと困る、と言う事を両立するために、レーザーカッターで穴を開けたアクリル板を用意し、背の高いコップなどをはめ込み式にした。
湯沸かし器や、コンロなど、危険なボタンが結構あって、こういったものを変に弄られないようにそのボタンやツマミにぴったりな形で回せなくするカバーを付けてくっつけている。まぁ壊されたりしたこともあったが。
握力が落ちてくるとちゃわんを持つのがつらくなる。そこで、ちゃわんにぴったりあって持つのをサポートするアダプタを作った。 市販のものも存在するけど、どうもしっくりこなかったので。ちょうどお茶碗に握り込める取ってがついたような形にした。
人間が立って使う事を前提にしているものは、ほとんどが車椅子からだと手が届かない。そう言ったものにアダプタを付けたり、紐を付けたりするためのアダプタとかを作ってはめている。
スタンドアップリフトはちょうど良いところを掴んでもらわないと姿勢が崩れてしまう。
そこで、ちょうど姿勢が安定するところに3Dプリンタで作った握りをつけ、そこを的確に握ってもらうようにした。特に親指が握りしめられるように凹凸を付けてあるので、安定するようになった。まぁ、なくてもそんなに問題があるわけじゃない部分だけど。
レーザーカッターで発泡ウレタンフォームの板に、小物がはまり込む形で切らせて、小物が確実簡単に整理でき、尚且つなくなったらすぐにわかるものが作れる。さらにこれをハードタイプのプラケースに入れてやると、完全オリジナルの薬ケースのできあがり。
通常は錠剤なんだが、漢方薬で飲み薬があるため、こんな物を作った。
アルマイト処理したアルミのプレートは、ある波長のレーザーを宛てるとアルマイト処理した表面だけが吹き飛ばされてアルミの地を出す事ができる。それにを塩素系漂白剤に入れてやると、アルマイトが剥がされた所だけが酸化して白くなって浮き上がってくる（入れすぎると全部参加して白くなる）。
こうしてやるとまるで金属彫刻をしたかのような仕上がりになる。これを利用して、金属製お名前キーホルダーを量産して付けてます。軍属かッ。 だけど相当な事がない限り絶対に忘れないいってのは大きいし、そう言う唯一無二の印を付けておくと忘れ物が少なくなります。
近くの工業試験場に公設のfablabが併設されていて、そこで一時間1000円とかでレーザーカッター使い放題（材料は持ち込み）、3Dプリンタは材料費だけで出し放題なので、ここを使っている。
3Dスキャナーも半日3000円とかで借りれるし、持ち出さない場合、レンタル中でなければ、1000円の中で使い放題。
データの作成は学生版の3D CAD (あ、ワイ放送大学の学生です) で作ってます。スキルは介護で離職する前から持ってるスキルです。
レーザーカッタの利用方法はもっと簡単で、精密でなくてよければ、紙にカットしたい線を書いてスキャンさせると、その線にそって同じようにアクリルやMDFを切ってくれるモードがあり、簡単なものはこれで作れる。
事例は3Dプリンタの方が多いけど、実際はレーザーカッターの方が出番が多い。ダイソーでMDFの板を買っていってカットとかよくやる。
3Dプリンタは自分でも持っていたけど、こちらを使う様になってからこちらの方が便利なので自分のものは使わなくなってしまった。
3Dプリンタばかり話題になるけど、実際には3Dプリンタよりもレーザカッターの方が手軽で強度もあって使う機械は多いと思う。自分で書いた線の通りに板を切ってくれる装置だって考えてみて。
